Doctors certificate for maternity pay

Question

I need to provide my mutuelle with a certificate in order for them to start the process of sending me papers for maternity pay. Does anyone know if this is a special type of certificate or just a letter? Also if it has to be from the gynae or can it be from GP? I don't have another gynae appointment until end of November so can't ask the doctor. Thank you

If there are many employees in your company your HR dept may have a prescribed format, else a letter (letter pad) from Gynaec would suffice.
However, the letter shall contain the due date of delivery and also state that you have completed 6 months.

Call your gynaec and inform him/her, she can post this to your address and you dont need to make an appointment for this:-)
